Cadets are required to fill out the following forms at the beginning of each school year.  

1. NJROTC Cadet Information Sheet

2. NJROTC Cadet Contract (requires parent/guardian signature)

3. Google Apps - Acceptable Use Policy (requires parent/guardian signature)

4. NJROTC Standard Release Form (requires parent/guardian signature)

5. FHSAA Preparticipation Physical (EL2) (requires parent/guardian and/or medical practitioner signature)

6. NJROTC Health Risk Screening Questionnaire (requires parent/guardian and/or medical practitioner signature)
