Cadet Registration Forms

Cadets are required to fill out the following forms at the beginning of each school year or within two weeks of joining the unit. Cadets shall not participate in any NJROTC activities outside class until all forms have been completed.

2. NJROTC Cadet Contract (requires parent/guardian signature)

3. Google Apps - Acceptable Use Policy (requires parent/guardian signature)

4. NJROTC Standard Release Form (requires parent/guardian signature)

5. JROTC Student Code of Conduct and Parent/Guardian Consent Form (requires parent/guardian signature)

6. NJROTC Health Risk Screening Questionnaire (requires parent/guardian and/or *medical practitioner signature)

7. FHSAA Preparticipation Physical (EL2) (requires parent/guardian and/or medical practitioner signature)

*NOTE: Health Risk Screening only requires medical practitioner signature if any question has a "yes" answer.
